Once you've printed and completed the forms for your FCC license application, use this checklist
to be sure you've covered all the details before sending your forms to Atlas.

1. Applicant Information Worksheet or
GPS Business Worksheet or
GPS Governmental Worksheet or
GPS Robotic Worksheet		 REQUIRED
2. License Guard (signature required) or
License Guard for GPS (signature required)		 REQUIRED
3. Authorization / Fee Calculation Form (signature required) REQUIRED
4. FCC Form 601 (signature required) REQUIRED
If paying by check, PLEASE mail the completed forms with your payment.
If paying by credit card, completed forms may be faxed with credit card information.
