Click on the format, MS Word (.doc) or Adobe (.pdf), relating to the form(s) you wish to
download.
A window will appear on your screen asking, "Would you like
to Open the file or Save it to your computer?" Click on Save.
A
"Save As" window will appear. You may "Save In" your
desktop using the default file name already in place in this window.
If the "Save In" and "Filename" info is satisfactory, click on
Save.
A Download Progress window will appear and when finished will
indicate: Download Complete.
(This time varies according to the speed
of your internet connection.) Once the download is complete, click the
Close button.
Clear the open windows from your Desktop. You should
have a "zip" folder containing your downloaded forms.
Double
click the folder, then double click the forms icon inside!
Forms in MS
Word format (.doc) will allow you to enter information on your computer and
print the form completed.
Forms in Adobe format (.pdf) only
allow printing of the blank form to be completed by hand.

the Word forms, please download the Adobe version. If you do not have an
Adobe Reader, it is available for free directly from Adobe! Just
